Expert assessment, rehabilitation and ongoing support for pelvic organ prolapse

Pelvic organ prolapse affects around one in three women at some point in their lifetime and the vast majority will never need surgery to manage it well. However, many women are given a diagnosis with very little explanation of what it means, what their options are, or what they can do about it.

We believe women deserve better than that. Our specialist Pelvic Health Physiotherapists combine advanced postgraduate training with extensive clinical experience, helping women understand their prolapse, manage their symptoms with confidence and make informed decisions about their care.

What prolapse can feel like

Prolapse presents differently in different women, but the most common symptoms include a feeling of heaviness or dragging in the pelvis, a sense of something coming down or sitting low, discomfort during exercise or at the end of the day, a noticeable bulge, changes to bladder or bowel function, or pain during intimacy. Some women have very clear symptoms. Others have a sense that something simply does not feel right.

Whatever brought you here, your concerns are valid and worth investigating properly.

Types of prolapse we treat

We provide specialist assessment and rehabilitation for all types of pelvic organ prolapse, including:

  • Anterior wall prolapse (cystocele or bladder prolapse)
  • Posterior wall prolapse (rectocele or bowel prolapse)
  • Uterine prolapse
  • Vaginal vault prolapse following hysterectomy
  • Enterocele
  • Combined or multi compartment prolapse

We support women at every stage, from women noticing symptoms after childbirth through to those experiencing changes around menopause and beyond.

Assessment that gives you real answers

Understanding your prolapse properly is the first step towards managing it confidently. Your appointment with us is detailed and designed to give you a clear understanding of what is going on, what is driving it and what can be done about it.

We assess the type and degree of prolapse, your pelvic floor function, the wider muscles and pressure systems that support it, your bladder and bowel function, your lifestyle and the activities you want to be able to do. You will leave with answers, not just a label.

How we treat prolapse

Conservative management, led by specialist physiotherapy, is recommended by NICE as a first-line treatment for prolapse. For many women, it is highly effective. A personalised rehabilitation plan can significantly reduce symptoms, improve function and help prevent prolapse symptoms from worsening over time.

Treatment typically includes pelvic floor rehabilitation, pressure management strategies, lifestyle advice, hands-on therapy where appropriate, pessary fitting and support to return to the exercise and activities that matter to you.

When conservative treatment is not enough

For some women, physiotherapy alone is not enough to manage symptoms fully, and a pessary offers an excellent next step. A pessary is a small, removable silicone device that supports the pelvic organs from inside the vagina. For many women, a pessary provides significant symptom relief and makes it easier to stay active, comfortable and confident

Our specialist pessary clinic offers expert fitting, a range of pessary types, and ongoing reviews to make sure the fit stays right over time. Your physiotherapist can refer you in directly if it is the right next step for you.

A complete prolapse pathway

Our prolapse service provides access to every stage of prolapse care under one roof. Specialist assessment, evidence based rehabilitation, pessary fitting and management, and onward referral to consultant urogynaecologists where surgical opinion is appropriate.

We work closely with urogynaecologists, gynaecologists and GPs across Wimbledon, Esher and the surrounding areas, so wherever your care needs to go, we can help it get there.
